The actions to take if an individual becomes distressed and unable to continue is to reassurance the individual and communicate with them that you understand their distress and tell them how well the individual is doing with the everyday skill. At my workplace I have experienced individuals becoming distressed on their first time to be allowed to go the shops after a long time being in the hospital. This could be due to what happened to them before and they are now afraid of being among people that they do not know. In such situation, I reassure the individual and communicate with them, giving the individual praises and how well the individual is doing and I will ask the individual if they want to continue. If the individual presentation appear to be more distressed and I will advise the individual to not continue with the leave. I will then record and report the senior how the individual presented themselves while on leave.
