An assessment is carried out with every new client that uses our services, this assessment must be done before the care package can start and the care plan typed up and in the property.
• When carrying out an assessment I am considering the following:
• Does the client have the capacity to be fully involved in the assessment process
• Are there any, Next Of Kin, Power of Attorneys or Advocates that need to present
• Would it be beneficial for the client for any other organisations to be present such as a social worker, district nurse, occupational therapist or GP
Once this has been established I will be assessing the client’s ability in the following areas:
• Hearing, sight and speech
• Well-being (any anxiety, agitated/ restlessness)
• Communication ability
• Mobility
• Medication ability
• Personal care needs
• Fluids and nutrition
• Finances
Once the above points have been considered any risk assessments will then need to be carried out.
